Who qualifies for Cancer Detection Program: Every Woman Counts

Our program provides FREE breast cancer screening and further testing to women who:

  • Do not have insurance
  • Have insurance but are unable to pay the deductible or co-pay required for service
  • 40 years of age or older
  • Have incomes at or below 200% federal poverty level

Our program also provides FREE cervical screening to women who:

  • Do not have insurance
  • Have insurance but are unable to pay the deductible or co-pay required for service
  • 25 years of age or older
  • Have incomes at or below 200% federal poverty level

Women's Health Partnership

The goal of our program is to decrease breast and cervical cancer mortality rates through outreach, education, and early detection strategies.

The four main components of the program are:

  • Maintaining an active community volunteer coalition
  • Monitoring quality clinical services
  • Providing professional education
  • Increasing public awareness through education and outreach

Diabetes Education Program

The Diabetes Education Program is a collaborative program that was created in response to the large number of people in our community who are living with diabetes. The Diabetes Education Program at Clinica Sierra Vista is committed to helping people control their diabetes and reducing long term complications. The key to managing Type 2 Diabetes is healthy eating, activity, self-monitoring blood sugars, medication, and stress management. The Diabetes Education Program provides group and individual counseling, education, and case management for diabetes. The Diabetes Education Program provides free Diabetes Classes on nutrition, medication, complications, diabetes management and lifestyle coaching in Spanish and English (Lamont and Arvin Community Health Centers). Case managers (available at the Lamont Community Health Center) help clients with diabetes make and maintain lifestyle changes.

Family Planning

Clinica Sierra Vista offers family planning services including free exams and contraceptive services for income eligible uninsured individuals.

Maternal Child Outreach Program (MCOP)

Clinica Sierra Vista is currently one of the largest providers of the Comprehensive Perinatal Services Program (CPSP). Since the initiation of the program, our patients have exhibited better outcomes than the area population from which they are drawn. The Maternal Child Outreach Program, referred to as MCOP, has three main components: medical care, patient education, and support services. The patient becomes part of a team which includes the doctor or nurse practitioner/midwife, nursing support staff, MCOP staff, nutritionists and counselors. The initial visit for the prenatal patient consists of a prenatal history taken by a program assistant in MCOP, followed by an enrollment in Women Infants and Children (WIC) and the physical examination. The program assistant and the nurse become the patient's case managers throughout the entire pregnancy, until her six-week postpartum check. Throughout the pregnancy, the patient is seen by the same medical provider, who may also offer her delivery services. Following delivery, the new mother is assessed in the hospital and given bed-side guidance as needed. Return appointments are scheduled at this time to ensure continuity of care. Home visits are also conducted.

In addition to the medical care component, the Maternal Child Outreach Program offers patient education and support services. All expecting patients participating in the MCOP program are offered an array of services including AFP screening, HIV information, antibody testing, counseling, nutrition services and early pregnancy and childbirth classes. These classes include not only labor and delivery preparation, but infant care, parenting and family planning. Family planning, including tubal ligation counseling, is also offered on a one-to-one counseling session basis.

The success of this program can be attributed to attentive case management and review of the prenatal patient's chart after each visit. Follow-up is conducted on every appointment that is missed. The Maternal Child Outreach Program (MCOP) is offered at all of Clinica Sierra Vista's primary care health centers.

Medical Nutrition Therapy

At Clinica Sierra Vista, our Registered Dietitians provide Medical Nutrition Therapy (MNT) services to patients at our health centers, who are referred to the program by their physician. MNT involves the assessment of nutritional status and the provision of diet, counseling, and/or specialized nutrition therapies to treat an illness or condition. MNT has been integrated into the treatment guidelines for a number of diseases, including cardiovascular disease, diabetes mellitus, hypertension, and obesity.

Medically Vulnerable Infant Program

Clinica Sierra Vista's Medically Vulnerable Infant Program (MVIP) is a nurse home visiting program for families of eligible babies just released from a Neonatal Intensive Care Unit (NICU) at local hospitals. The NICU experience can be overwhelming for many parents. It may be difficult for them to understand their infant's condition or medical problems. Many of the families find it hard to cope with their infant's special needs when they go home. They may have questions that didn't arise in the hospital setting. They may not know where to go to get the information they need.

Clinica Sierra Vista offers home based services to children (until 3 years of age) who meet the medical criteria for the program. The program offers home based Registered Nurse-Case management services, which include:

  • Intensive home visiting
  • On-going developmental assessments
  • Infant assessments
  • Education on infant care, safety, child development and their infant's special needs
  • Support and encouragement
  • Facilitating health care coordination
  • Providing referrals and resource information to link families to valuable community services

Registered Nurse visits can provide families with guidance to empower parents to become better advocates for their infants and children. MVIP services are intended to:

  • Help families establish medical homes
  • Encourage up to date immunizations
  • Link families to community resources
  • Reduce hospitalizations and emergency room visits
  • Prevent or minimize developmental disabilities or delays
  • Educate and support families in regards to their infant's special needs
  • Reduce infant mortality in this high -risk population
  • Prevent child abuse

The Medically Vulnerable Infant Program is a free service for those children who meet the criteria and parents consent to participate voluntarily.
