Open enrollment begins October 15, 2018, and runs through January 15, 2019. You must enroll by December 15, 2018, to ensure your benefits will kick in on Jan. 1, 2019. What to Bring With You
When visiting the health center for the first time:
- Please bring proper Identification, which may include a Social Security card, valid driver’s license, birth certificate or another form of identification, if you have it.
- Bring your Medi-Cal card, Medicare card or private insurance card.
- If uninsured, we offer a sliding fee discount based on your family size and finances. Please bring a pay stub, an unemployment check stub, a recent tax return, or a letter from your employer to prove your family income.
Clinica Sierra Vista accepts Medi-Cal, Medicare, most private insurance, most HMOs, senior and commercial plans and cash. We have Certified Enrollment Counselor from Covered California and Medi-cal at our health centers who will assist patients with enrolling in these programs, and we offer a sliding-fee scale for the uninsured.
